It is much easier, and often faster, to do things for people rather than watch them struggle with it themselves. 
An illness or disability can lead to a change in a person's ability to perform a task. 
Doing a task for a client promote dependence and can lead to decline in function, however. 
Nurses generally ask themselves these three questions: 
"What do I have to do for this person?"
"How can I help this person to do more for himself or herself?"
"What can this client do for himself/herself?"
